Speech Therapy CPT Codes: 92507, 92521–92524, and More

✓ Reviewed against ASHA CPT coding resources, ASHA: Medicare Coding Rules for SLP ServicesEvidence level consensusLast reviewed October 14, 2026Published October 14, 2026

CPT codes describe the service an SLP provides; ICD-10 codes describe the diagnosis. The main speech therapy codes are 92507 (individual treatment), 92508 (group), and four evaluation codes that replaced 92506 in 2014: 92521 (fluency), 92522 (speech sounds), 92523 (sounds plus language), 92524 (voice).

What CPT codes are

ASHA explains that the CPT coding system describes how to report procedures or services, is maintained and copyrighted by the American Medical Association, and uses five-digit codes such as 92507. CPT codes pair with ICD-10-CM diagnosis codes, which ASHA also publishes annually for speech-language pathology.1,3

Coding rules worth knowing

  • Most SLP codes are untimed and billed once per day regardless of session length; timed codes include a time in the descriptor and can be billed in units.
  • 92522 and 92523 should not be billed together.
  • SLPs bill 92507, 92508, and 92526; codes such as 97110, 97112, 97150, and 97530 are generally performed by physical or occupational therapists.
  • Coverage and coding policies vary by payer; Medicare rules are often adopted by other payers.

These rules come from ASHA’s CPT and Medicare coding guidance.1,2

Frequently asked questions

What is CPT code 92507?+

Treatment of speech, language, voice, communication, and/or auditory processing disorder — individual. ASHA notes SLPs may also use it for auditory (aural) rehabilitation.

What is the CPT code for a speech therapy evaluation?+

It depends on what is evaluated. 92521 covers speech fluency (stuttering, cluttering); 92522 covers speech sound production (articulation, phonological process, apraxia, dysarthria); 92523 covers speech sound production plus language comprehension and expression; 92524 covers behavioral and qualitative analysis of voice and resonance. They replaced 92506 on January 1, 2014.

What is the difference between 92522 and 92523?+

92522 is an evaluation of speech sound production only. 92523 adds evaluation of language comprehension and expression. ASHA notes the two should not be billed together, and that a language-only evaluation uses 92523 with modifier -52.

What is the difference between CPT and ICD-10 codes?+

ICD-10-CM codes describe the diagnosis, such as F80.0 for phonological disorder. CPT codes, maintained by the American Medical Association, describe the procedure or service provided, such as 92507 for treatment. A claim usually needs both.
